SPECIFIC PSYCHIC DISORDERS

Summary
Changes in the prevailing views on war have led to a new appreciation of wartime stress. This might explain why a generally accepted diagnostic category for disorders attributable to the late psychiatric impact of wartime stress was not introduced until 1980.
The origin and clinical characteristics of the post-traumatic stress disorders are discussed; the limitations of this diagnostic category are also considered.
In the post-war years the mental problems of war victims were presumed to result from the physical hardships endured. Only gradually was the significance of severe psychological traumatization understood. The successful adaptation of many war victims was impeded by social bias and denial. The magnitude of the late psychiatric consequences was greatly underestimated. War victims constitute an exceptionally high-risk group. There are indications of shortcomings in the medical and legal support provided since the end of the war.
In the past conflicts and armed skirmishes between states and nations were so frequent that acts of war were a normal phenomenon of human life. Not until the 19th and 20th centuries did this change. The potential for destroying whole armies increased greatly as a consequence of advancing technological developments. With the advent of nuclear weapons the devastating force had increased so much that an armed conflict in which atomic weapons are used would be a direct threat to the continued existence of mankind. Parallel to these developments in military technology was a change in attitude: people began to consider war as an unnatural phenomenon, as an anomaly. In this century we of the Western world generally think of war as being abnormal and unacceptable (1).
As a consequence of the change in attitude towards the role of armed combat at an organized level, appreciation of wartime stress has also become radically different. Whereas, in the past, wartime stress was considered an unavoidable side-effect of a normal occurrence, today the emotional burden on soldiers in combat as well as civilian victims is considered most uncommon.
In the new diagnostic system for psychiatry introduced in 1980, acts of war are included among the experiences that fall outside the range of normal human experience (2,3). Along with views on the significance of war for the community, scientific insights into the immediate and delayed emotional consequences of war have also been modified.
