SUMMARY
In this study we tried to develop a methodology which enabled us to visualize the psychotherapeutic process. Doing so, we were able to clarify something about the relation between the theory of practising psychoanalytic psychotherapy and the actual conduct of psychoanalytic psychotherapy. That is why we investigated a number of short-term and time-limited psychoanalytical treatments.
First we considered whether such short-term psychoanalytic psychotherapy as D.H. Malan proposed is a specific form of verbal psychotherapy or not. We demonstrated that, in comparison with Rogerian treatment, Malan-therapy is a specific form of psychotherapy. Malan-therapists were more interpreting and more actively guiding the therapeutic process than their Rogerian colleagues were. On the one hand, we examined 27 complete sessions from 3 Malan-therapies and 162 5-minutes fragments from 12 Rogerian therapies on the other. We used the Strupp multidimensional system for analyzing psychotherapeutic communications.
Secondly, we tried to visualize the psychotherapeutic process within Malan-therapy. By means of a specific content analysis system (relevant for the psychoanalytic frame of reference), we studied 94 complete therapy sessions from 7 analytically trained psychotherapists. They did all in all, 12 therapies in the way Malan prescribed. Malan proposed his type of treatment as a form of expressive psychotherapy. He claimed that the interpretation of the transference in terms of early objectrelations (TP interpretation) is the most effective instrument in this form of treatment.
Our content analysis system includes categories about the analysis of transference and resistance. Apart from these categories for the interventions of the therapists, the system also includes functional categories for the remarks of the patient (as ‘insight’ or ‘resistance’). Finally a number of categories, regarding affects related to termination in treatment (for example ‘aggression’ or ‘grief’) were included. We used frequential and sequential statistical procedures in this research program.
We demonstrated that short-term and time limited-psychoanalytic psychotherapy as proposed by Malan is not a form of expressive psychotherapy. Rather it is a form of supportive and explorative psychotherapy with periods of interpreting inside. Working through the transference or resistance is done in a limited way. The genetic transference interpretation is lacking. Therapists are staying close to the experiencing ego of the patient, they are focused on the here-and-now of the therapeutic situation.
Malan-therapy is a form of psychoanalytic psychotherapy in the way Malan-therapists are using psychoanalytical concepts and insights about personality structure, psychopathology and conducting treatment. It is not psychoanalytical in the sense of systematically working through transference manifestations or defense structures as done in expressive psychotherapy or psychoanalyses proper. It is a supportive form of psychotherapy with periods of interpreting inside.
Our findings are in conflict with Malan’s and with the research findings of, for example, Marziali (1984). Our findings are, however, on line with those of Ferenczi and Rank (1925), Alexander and French (1946), and more recently with the findings of Wallerstein (1986).
